Please include the name on the card, the number and expiration date, the billing address where the credit card statement is received, the security number on back of card, email and a telephone number.
  

Cancellation Policy for workshop tuition:
All cancellations must be received in writing prior to the start of the conference.

REFUNDS: An 80% refund for cancellations before July 15^th; and 50% refund between July 16-September 23^rd.  No refunds for cancellation within 24-hours of the start of conference.
